61-year-old patient with triple vessel disease and diabetes successfully treated with advanced off-pump coronary artery bypass surgery (OPCAB)
Hyderabad, August 11th, 2026: Doctors at Yashoda Hospitals, Hyderabad, have successfully performed a rare and highly complex beating-heart triple bypass surgery on a 61-year-old patient from Wanaparthy district who was diagnosed with critical left main coronary artery disease, triple vessel coronary artery disease, and type-2 diabetes. The patient had been experiencing recurrent episodes of left-sided chest pain for nearly six months. Detailed cardiac evaluation revealed severe blockages in the left main coronary artery and multiple major blood vessels supplying the heart, placing him at very high risk of a major heart attack, heart failure, and sudden cardiac death.
Given the severity of the condition, the cardiac team recommended immediate bypass surgery.
The procedure was performed by Dr. Vengal Reddy S., Consultant Cardiothoracic & Vascular Surgeon, using Off-Pump Coronary Artery Bypass Grafting (OPCAB ×3), a technique in which bypass surgery is carried out on a beating heart without using a heart-lung machine.

Speaking about the surgery, Dr. Vengal Reddy said, “Patients with left main coronary artery disease and triple vessel disease are at extremely high risk of major cardiac events. Performing bypass surgery on a beating heart requires a high level of surgical expertise. Timely diagnosis, careful planning, and early intervention were key factors in achieving an excellent outcome in this case.”
Three bypass grafts were completed successfully. The surgery was further complicated by the need for a coronary endarterectomy on the diseased segment of the right coronary artery, making the operation particularly challenging. The procedure was completed without any major intra-operative complications.
Following surgery, the patient was shifted to the cardiac critical care unit, where he received ventilatory support, haemodynamic monitoring, diabetic management, physiotherapy, and intensive CT-ICU care.
Dr. Kaladhar S., Senior Consultant & HOD, Critical Care Medicine, said, “Continuous CT-ICU monitoring, respiratory support, and coordinated multidisciplinary care played a critical role in the patient’s recovery after this high-risk cardiac surgery.”
The patient showed steady improvement, was extubated on the first post-operative day, mobilised early, and was later discharged in stable condition.
This case is significant because it involved critical left main coronary artery blockage along with triple vessel disease, and the patient was successfully treated using a rare beating-heart triple bypass procedure. The surgery was made even more complex by the addition of coronary endarterectomy, an advanced cardiac surgical procedure.
