Manipal Hospital Introduces India’s First AI-Powered Elekta Evo System for Adaptive Radiotherapy

Manipal Hospital, Yelahanka, Bengaluru has become the first hospital in India to install the Elekta Evo, an advanced AI-enabled adaptive CT-Linac platform that brings Online Adaptive Radiotherapy (oART) into routine clinical practice. The new technology allows radiation treatment plans to be modified in real time based on daily changes in a patient’s anatomy, helping improve treatment precision and patient outcomes.

The installation comes after Elekta showcased the Evo platform during AROICON 2025, India’s leading radiation oncology conference held in Kolkata. The hospital has already begun using the system to treat patients requiring advanced radiation therapy.

The need for such technologies is increasing as India’s cancer burden continues to grow. According to the Indian Council of Medical Research (ICMR), nearly 1.46 million people are diagnosed with cancer each year in the country. With many patients presenting at advanced stages of the disease, precision-driven radiation therapy has become increasingly important for achieving better clinical outcomes while protecting healthy tissue.

At Manipal Hospital, the new platform has strengthened the hospital’s radiation oncology services by enabling doctors to tailor treatment throughout the therapy cycle. The technology allows clinicians to review anatomical changes before each session and adjust radiation delivery accordingly, making personalised treatment more practical without affecting clinical efficiency.

The hospital has already completed its first online adaptive radiotherapy treatment using Elekta Evo on a patient with locally advanced rectal cancer. Following neoadjuvant chemoradiotherapy, the patient experienced minimal radiation-related side effects, demonstrating the potential of adaptive treatment to accurately target tumours while reducing radiation exposure to nearby healthy organs.

The Elekta Evo platform combines Iris™ high-definition AI imaging with online and offline adaptive radiotherapy capabilities, allowing clinicians to visualise tumours and surrounding organs with greater clarity. The system also supports multiple radiation therapy techniques through a single platform, helping deliver accurate doses while limiting unnecessary radiation to healthy tissues.

Adaptive Radiotherapy (ART) differs from conventional radiation therapy by allowing treatment plans to be updated whenever changes occur in a patient’s anatomy or tumour during the treatment course. Elekta Evo integrates Elekta ONE® Online software with AI-assisted contouring, faster dose calculations and distributed treatment planning, enabling quicker and more efficient clinical decision-making.

Currently, Manipal Hospital is using the Elekta Evo system to treat patients with rectal, cervical and prostate cancers through online adaptive radiotherapy. Initial clinical observations indicate that patients have tolerated the treatment well, with minimal treatment-related side effects reported during the ongoing therapy.

The introduction of India’s first Elekta Evo system marks an important advancement in the country’s radiation oncology landscape.
